The Hospitallers are the most skilled and compassionate physicians in the entire Imperium. Frequently accompanying the Imperial Guard on campaigns, they are perhaps the most beloved organization in the eyes of the common Imperial citizen. Yet, this is the Imperium, where everyone harbors a darker side; the Ecclesiarchy and the Inquisition frequently requisition Hospitallers to assist in the interrogation of prisoners.

Just like their medical expertise and surgical skills, they serve as a beacon of faith for those in need. They offer prayers to salvage zealous souls and recite creeds of fortitude, reminding the wounded in makeshift wards of the steadfast saints who endured immense suffering. With unyielding grace, they bear great hardships. They whisper revitalizing litanies to ease the pain of the wounded and lift the veil of exhaustion.

During surgery, even as they extract burning rounds, crystallized shards of sorrow, and tainted shrapnel from screaming soldiers, the Hospitallers spit curses of hatred against heretical creeds.

The Orders Hospitaller and the Emperor's Mercy

To fulfill this duty, they may administer lethal toxins instead of stimms and painkillers (though administering stimulants to the wounded for euthanasia seems rather odd, as modern practice typically employs potassium ions and anesthetics).

When an Astra Militarum soldier is too grievously wounded on the front lines, beyond the reach of all medical aid, or infected by a virulent plague, they will use specialized elixirs or their bolt pistols to grant the soldier final rest—the Emperor's Mercy.

Should such a terminal state occur within a field hospital, a Hospitaller Sister will carry out this duty, a practice depicted in the short story *Duty's End*.

The Sisters utilize a colorless liquid—resembling alcohol, yet potent enough to etch a glass gray—to end a patient's suffering. It is a method far removed from conventional euthanasia. Of course, if a patient can still be saved, the Hospitallers will provide medical assistance far more professional than standard Astra Militarum medics.

Thus, it is no exaggeration to say that a Hospitaller can indeed end a life with a single injection.

If a soul is beyond salvation or has fallen to corruption, the medical staff will deny them such mercy. Instead, they will terminate the corrupted life without hesitation, and with utter cruelty.

Frequently deployed to war zones, the Hospitallers rush to the front lines equipped with specialized medical gear. These tools, while meant to treat the wounded, can also be turned against the enemies of the Imperium.

Sisters of the Orders Hospitaller are transported aboard sanatoria barges to hospices in hive city slums or to exploratory missions on the fringes of the Imperium, where their skills undergo extensive practical training.

Through countless battles, they grow accustomed to the brutal realities of war and gain a profound understanding of the grim laws of survival.

The Hospitallers and Their Equipment

(1) The Chirurgeon and Hospitaller Medicae Tools

The illustration above depicts a Hospitaller equipped with a Chirurgeon.

While lore databases like Lexicanum and Fandom primarily focus on the version worn by Fabius Bile, the information here is synthesized from various sources. The Chirurgeon is an automated battlefield surgical apparatus widely equipped by high-ranking Hospitallers and Space Marine Apothecaries. Due to its rarity and formidable functionality, it is reserved exclusively for fully qualified Sisters. The apparatus incorporates complex auto-diagnostic systems and highly automated wound-preprocessing systems, allowing the wearer to gather extensive trauma data without manual inspection, while automatically performing wound debridement, disinfection, and the excision of damaged tissue.

The Hospitaller's surgical suite can rapidly cut through the light carapace armor worn by the Battle Sister shown above.

However, the backpack unit of the apparatus does not include power tools capable of shearing through the thick power armor of a Space Marine. Consequently, when treating a Space Marine, the wearer must utilize hand saws and power drills to quickly remove sections of the armor, which is often warped and mangled by the sheer force required to fell an Angel of Death.

In principle, the apparatus does not feature equipment for harvesting or preserving gene-seed; thus, this task must still be performed by an Apothecary. Because gene-seed is of paramount spiritual and practical importance to a Chapter, Space Marines will not entrust its recovery to outsiders unless absolutely necessary. (Hence, she truly possessed no preservation equipment at the time; otherwise, the protagonist's gene-seed would undoubtedly have been harvested.)

The Hospitaller's medicae tools consist of the finest battlefield medical implements. This kit contains a diverse array of sacred oils, unguents, surgical instruments, and sterilizing agents to aid those wounded in combat.

(2) Hospitaller Carapace

Sisters of the Orders Hospitaller typically serve in active war zones, requiring far greater protection than other non-militant orders. The distinct carapace armor they wear is one of the most welcome sights to wounded Guardsmen and Ecclesiarchy faithful alike, symbolizing a merciful angel sent by the God-Emperor Himself to bring healing.

The Hospitaller's armor is fundamentally derived from the same design as that of the Battle Sisters, though it is modified to be lighter and more agile.

Beyond the protection offered by the plating itself, their cowls are frequently treated with incense and unguents, while the rest of the armor is sealed and chemically proofed. Similar to their Battle Sisters, this armor can be fitted with a sealed rebreather helmet to withstand the most hostile environments.

(3) Chaplet-Ecclesiasticus

The Dark Angels are not the only ones who associate repentance with beads. Hospitallers wear necklaces strung with adamantium beads. Each bead represents an act of penance by the Sister, symbolizing one, ten, or up to a thousand acts of contrition depending on the wearer's preference.

(4) Ring of Suffrage

The Ring of Suffrage is a specially crafted, slightly twisted ring that causes mild discomfort when manipulated. This design is intended to remind the Hospitallers to adhere to corporal mortification (implying, perhaps, that the mind remains free? By the Emperor!), reminding them of the heavy burdens they bear as citizens of the Imperium, as well as the trials and tribulations endured by the Emperor and His saints.

Similar to prayer beads or a crucifix, the term "suffrage" itself denotes intercessory prayer. Manipulating the ring functions much like a Tibetan prayer wheel—each turn represents a brief prayer. After all, reciting formal litanies is often too time-consuming and requires a proper setting, whereas Hospitallers are typically deployed on the battlefield.
